New Starter assessments

New Starter Clearance Questionnaires:

These are clinical questionnaires designed to be completed either in paper format and returned to PHC in freepost envelopes, or to be completed by the employee on line, via the website portal.

These questionnaire can be tailored to your requirements and can have supplementary clearances such as food handler clearance or night worker clearance attached as you wish.

Typically the outcome is reported to the manager within 48 hours of the completed questionnaire arriving at PHC House. If necessary the candidate is contacted by telephone in order to discuss statements made on the forms. The final report will indicate whether the candidate is fit, whether they have a medical condition which is likely to be covered by the disability aspect of the Equality Act 2010 and any reasonable adjustments that the employer should consider.

Physical Assessment for New Starters:

Some employers have a requirement for a full physical assessment to take place, these medicals may include health surveillance elements as baseline records such as hearing tests, hand arm vibration assessments. These medicals can be done at any of our clinics or on-site by our nurses and technicians. Again, these will be reported as fit, whether they have a medical condition which is likely to be covered by the disability aspect of the Equality Act 2010 and any reasonable adjustments that the employer should consider.
